Setting up a poll...

Ok, I really know nothing about php.
1. Could I get some help with setting up this script: http://www.free-midi.org/scripts/simpoll/simpoll_v_1_0.zip
2. Do I just need to learn php to set it up and/or modify it? If so how would you suggest I learn?
3. If you think I shouldn't use this polling script then what do you think I should use?

What exactly do you need help with? What part of the readme? I know a little php.

Its just that I know nothing... What would I enter into my basic html document? And what do I do with everything else? As I said I know nothing... A step by step explanation would be AWESOME but I understand if you don't have time or plane just don't want to.
